REGISTRATION CHECKLIST: I have enclosed separate REGISTRATION FORMS for each child.
I have included REGISTRATION FORMS-1 & 2 and enclosed a CHECK/MONEY ORDER PAYABLE TO HOMER SCHOOL DISTRICT 33-C.
If I am an IN-DISTRICT RESIDENT, I have enclosed REGISTRATION FORM-3, INDICATING THE TRANSPORTATION OPTION FOR MY CHILD.
My child’s registration COURSES reflect his/her GRADE AS OF THIS MAY 2007.
I have selected my child’s 1st, 2nd, and 3rd CHOICE for SELECTED SESSION(S).
IMPORTANT REMINDERS:
**Students will be ASSIGNED ON A FIRST COME, FIRST SERVED BASIS.
**CONFIRMATION OF REGISTRATION will be mailed home along with a schedule indicating session and course, assigned bus, teacher, and room number.
